Ohio Shared Living offers adults with developmental disabilities the option to live in a supportive home with a family member or caregiver who provides at least 20% of their care. It’s a personalized, home-based alternative that promotes stability, independence, and connection.

community resources.
A service that offers learning and volunteer opportunities to build skills for community employment aligned with a person's interests, strengths, and abilities.
This service includes help with things like getting dressed, making food, and accessing things in the
community.
A coordinated service that provides support, supervision, and care to ensure members' health and well-being in the community, while promoting independence and assisting with daily living needs.
From our fully commercial kitchen, health department inspected; hot meals are provided during day program, prepped meals, plus delivery available. Support services Include training in meal preparation as part of daily living skills.
Non-Medical Transportation assists a person with transportation to their day services, employment
services, or places of employment.
One-time household start-up costs for someone moving from an intermediate care facility (ICFIID) or a
qualifying nursing home stay and enrolling on an IO waiver.
Medication administration training and certification developed by the Department of Developmental Disabilities (DODD) authorizes our staff to safely administer meds and perform a variety of tasks for members with different medical conditions While here at Phazes.
